Paciente hemodinamicamente estable Embarazo ectópico no roto Sangrado transvaginal leve Sin dolor o con dolor leve BhCG <5 000 mU/mL Ultrasonografía: masa ectópica con diámetro 40 mm Embarazo menor de 3.5 cm Ausencia de frecuencia cardiaca embrionaria Pruebas hematopoyéticas, hepáticas y renales sin alteraciones.
Metotrexate (MTX)
La dosis: 1mg/kg o 50mg/m2.
Multidosis 1 mg/kg, días 1, 3, 5 y 7
Dosis única 50 mg/m2 1er día
-Edad >35 años -Uso de duchas vaginal -Inicio precoz de la actividad coital (<18 años) -Trasnferencia intratubarica.
-Uso actual de Anticonceptivos orales con estrógeno/progestina - Alteraciones en la concentración de progesterona -ETS previa - Infección por Chlamydia trachomatis o por Neisseria gonorrhoeae. (30 -50%) -Proceso inflamatorio previo -Parejas sexuales >1 -Tabaquismo -Infecciones pélvicas y abdominales tales como apendicitis -Aborto espontáneo previo
-Patología tubaria subyacente es probablemente la vía común para el embarazo ectópico inicial y subsecuente. -Embarazo ectópico previo -Alteración tubárica, anomalías congénitas, endometriosis, salpingitis itsmica nodosa, miomas intrauterinos. -Procedimientos de esterilización fallidos -Enfermedad pélvica inflamatoria (EPI) -Antecedente de cirugía tubaria -(OTB), cirugía pélvica (cesárea, ooforectomía), cirugía abdominal - apendectomía. -Uso de DIU -Fertilización in vitro en embarazo actual.
