Recommend = buy inoculators that are compatible with vaccine with longer shelf life and less sensitivity to temperature variations

Condition = gas leak

Criteria = inoculators should not leak

Effect = temp not maintained

Criteria = should be properly maintained

Effect = vaccines are impotent

Cause = gas leak

Recommendation = Ministry officials should consider the feasibility of replacing the old inoculators with the new portable refrigeration units for transporting vaccines in the field

Cause = careless handling and poor maintenance

Recommendation = have more qualified technicians = strict penalties for poor handling

Cause = lack of scheduled maintenance

Recommendation = schedule for regular maintenance and repairs of the inoculators should be drawn up and implemented by the Department of Logistics
