Goal of the Post Secondary transition meeting
Assists identified students, parents and teachers plan for a successful transition when they graduate form TCDSB
This includes planning for post secondary opportunities and /or community participation supports
PPM 140
PPM 140 is a Ministry of Education policy statement about incorporating methods of Applied Behaviour Analysis (ABA) into programs for students with Autism Spectrum Disorders.
Who is eligible for the Transition Meetings?
It is for students in a High School Setting and identified as havinga :
Multiple Exceptionality (ME)
Development Disability (DD)
Mild Intellectual Disability (MD)
Autism Spectrum Disorder (ASD)
How do meetings take place?
The meeting is initiated by the school
The meeting is documented in the form of Case Conference
Parent (s)/guardians(s) must be present and the student is encourage to attend, as appropriate

5. ODSP application (student to apply at 17.5 years old)
6. Encourage work experience and regular outings in the community (WET-P)
7. Connect to agencies and community supports

3.Increase Self Advocacy & self Regulations skills
4. Follow up with DSO process

Plan for transition between various activities and setting involving students with ADS

1.Continue to increase indepence, self advocacy & self regulations skills.
2. DSO and ODSP should be omplete
3. Increase community involvement and community supports
4. Encourage family to visit specific programs and possibly begin a slow transition if appropriate
5. Graduation planning
6. School to provide family with reports and assessments for portfolio if needed
7. Encourage parent netwoking
